If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

BSO Family Practitioner Services has developed desk aids for both GP practices and patients, which covers the majority of cases.  Please see link below.  The aim of providing the aids is to set out what the patient is required to provide and make this as widely available as possible. BSO web page

We will ask you to complete a health questionnaire  which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients will be offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. You should provide a sample of urine at this appointment.

Medical treatment is available from the date of registration. Please contact reception for further information.

Changes to blood and organ donation preferences

From 30th September 2021, patient are advised that their organ donation

preferences can no longer be processed through their GP practice, but should

they wish to, they can record their preferences directly through the organ

donation online registration website, or directly by phone.
